Ready to pre-register? 

Please go the to MyDRS portal to pre-register! You need to be a registered Temple University student to use this form. If you are a prospective student and are interested in learning more about the services we offer, please email us! Once you complete the form, you will be assigned a coordinator, who will reach out to you within 48 business hours to set up a registration appointment.

Documentation

Upload any documentation that you have available to you. We accept notes from doctors, mental health counselors, physical or occupational therapists, IEPs, etc. Please be sure that the documentation includes the letterhead of the office. Screenshots from a health portal can be helpful, but they're not considered formal documentation.
